Equality Monitoring Form

Severnside Institute for Psychotherapy has a policy to carry out Equality Monitoring to help us reach and serve more diverse communities than we currently do.

As part of this, we ask those who are comfortable doing so to complete our Equality Monitoring Form. Participation is optional, and all responses remain completely anonymous and are not connected in any way with personally identifiable information.
