Psychoanalytic accounts of gender variant patients in the literature are rare. Psychoanalysis with these patients suffers from historical biases that determine short-sighted and pathologising interpretations of gender diversity. It is however possible to take an open and affirmative approach to gender reassignment during the crucial years preceding the decision to undergo gender reassignment surgery. This can be done by considering the patient’s identifications and dreams as a developmental process of their transition path, and analysing transferential and counter-transferential patterns in the therapeutic relationship.
